If you are on any of the blood thinners (anticoagulants) on the following downloadable list please discuss this with our office and your prescribing physician. You MUST get the authorization of the prescribing physician before discontinuing the use of prescription medications.

For your upcoming injection at Greenwood Ambulatory Surgery Center, you will need to bring the following:

  • Downloadable paperwork, printed and completed
  • Insurance cards or Worker’s Comp/Auto Information
  • Driver’s license or other government issued identification
  • All patients with commercial insurance are expected to bring in payment at check-in in form of check, credit card, or exact cash. These documents are required for each visit to our facility.

    The GASC nurse line is available 24 hours a day with pre-operative instructions. Please call 720.493.4109 for instructions for your injection.
